How do I file for full custody?

The mother of my child has had full custody for the last three years; I recently went to court and was granted immediate temporary custody. I now need to file for full custody, and am not sure how to do this. Since there was already a custody order filed in the court where the mother was the petitioner, and I was the respondent, do I fill out this new paperwork to establish custody as the respondent? Or since it is a new custody claim, do I fill it out as the petitioner?
