Do I have to provide child support if the child does not have my last name?
I have a 5 year old daughter. My daughter does not have my last name, nor am I on the birth certificate. I make 10.50 an hour. Will I have to pay child support if we go to court?

Yes, regardless of birth certificate or last name. If you are the biological parent you will be forced to pay child support, at the rate of 18% of your gross income or a minimum of $100 per month even if you have no income at all.
